Job Description

Orchard Manor, a 121-bed skilled nursing facility, is seeking an experienced and detail-oriented RNAC or LPNAC to join our team. We prefer a candidate with strong MDS experience who can effectively coordinate and complete resident assessments in compliance with state and federal regulations. If you are organized, knowledgeable in long-term care reimbursement and assessment practices, and committed to quality resident care, we would love to hear from you. Position Summary The
RNAC / LPNAC
is responsible for conducting, coordinating, and completing MDS assessments in accordance with state and federal guidelines. This role helps ensure accurate documentation, regulatory compliance, appropriate reimbursement, and high standards of resident care. Key Responsibilities Conduct and coordinate MDS assessments according to state and federal regulations Ensure timely and accurate completion of all required assessment documentation Maintain knowledge of current MDS/RAI guidelines and regulatory requirements Support compliance with insurance and HMO documentation requirements Collaborate with interdisciplinary team members to gather assessment data Help support accurate reimbursement processes related to resident assessments Apply working knowledge of PDPM in assessment coordination and documentation Monitor assessment schedules and submission deadlines Promote quality outcomes for residents through accurate clinical assessment practices Qualifications Current RN or LPN license in good standing Experience as an RNAC or LPNAC in a skilled nursing or long-term care setting preferred Strong knowledge of MDS/RAI regulations Familiarity with insurance and HMO requirements Working knowledge of PDPM preferred Strong organizational skills and attention to detail Ability to work collaboratively with residents, families, and interdisciplinary teams Why Join Orchard Manor At Orchard Manor, we truly care about our residents and employees.
