Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Home Health team. In this role, you will provide skilled nursing care to patients, families, and caregivers in compliance with Medicare Home Health Conditions of Participation, Pennsylvania licensure laws, and agency policies. You will play a vital role in ensuring timely and accurate documentation, collaboration with team members, and delivering patient-centered care.
Primary Responsibilities:
Patient Care:
Deliver quality Home Health services safely and effectively. Implement patient care plans as directed by the RN and evaluate patient responses. Administer medications according to the PA Nurse Practice Act. Provide patient education and support to promote self-sufficiency. Monitor and report changes in patient conditions to the RN Case Manager or Clinical Manager.
Coordination and Documentation:
Collaborate with team members to ensure continuity of care. Submit accurate clinical notes and maintain proper documentation in compliance with agency guidelines. Assist patients with daily living activities (e.g., bathing, feeding) as required.
Resource Utilization:
Manage caseloads and coordinate travel time to ensure cost-effective care delivery. Participate in evening/weekend rotations as required.
Customer Service:
Demonstrate empathy, respect, and professionalism in all interactions. Maintain accountability for patient satisfaction throughout the care episode.
Performance Improvement:
Contribute to quality improvement initiatives and participate in clinical record reviews. Serve on committees and assist with special projects as requested.
Qualifications:
Graduate of an accredited practical nursing program. Current Pennsylvania LPN license. Minimum of one year of nursing experience (community health, home health, medical/surgical, or critical care preferred). Valid driver's license, reliable transportation, and automobile insurance. Current CPR certification. Ability to work effectively in a Home Health setting and handle stress. Satisfactory background check and professional references.
Physical Requirements:
Visual and hearing ability sufficient for communication and patient care. Manual dexterity for writing, keyboarding, and telephone use. Ability to work in varied home and facility settings.
Working Conditions:
