Position Summary The Licensed Practical Nurse (LPN) provides skilled nursing care under the direction and supervision of a Registered Nurse and in accordance with physician orders, agency policies, federal regulations, and the Pennsylvania Nurse Practice Act. The LPN assists with implementing and evaluating patient care plans, monitoring patient conditions, and providing education that supports optimal patient health and well-being. Essential Duties

Comply with all applicable laws, regulations, agency policies, and professional nursing standards.

Maintain an active Pennsylvania Licensed Practical Nurse (LPN) license.

Provide skilled nursing care under the supervision of a Registered Nurse.

Assist the Registered Nurse in carrying out the patient's individualized Plan of Care.

Deliver patient-centered care according to established professional standards and physician orders.

Support patients in achieving maximum independence, function, and quality of life.

Key Responsibilities

Patient Care

Assist the Registered Nurse with specialized nursing procedures as permitted under the Pennsylvania Nurse Practice Act and physician orders.

Participate in patient assessments, data collection, treatments, and interventions as delegated by the Registered Nurse.

Assist with the development, implementation, and ongoing evaluation of patient care plans.

Monitor patient health status during each home visit and report significant findings promptly.

Verify and reinforce medication adherence and prescribed medication regimens.

Collaborate with the healthcare team to ensure patient needs are addressed and care plans remain current.

Reinforce patient and family education related to the Plan of Care, medications, treatments, disease management, safety, and overall wellness.

Communication & Documentation

Prepare and maintain accurate clinical notes, visit records, and progress documentation according to agency requirements.

Communicate patient status changes and concerns to the supervising Registered Nurse.

Adhere to agency policies and procedures regarding communication and documentation.

Assist with patient referrals and coordination of services when appropriate.

Teamwork & Professional Development

Participate in staff conferences and interdisciplinary care discussions.

Support a collaborative team environment and contribute to positive patient outcomes.

Attend orientation programs, in-service training, continuing education, and other required professional development activities.

Take ownership of professional growth through ongoing education, workshops, conferences, and learning opportunities.

Maintain current knowledge of nursing best practices and applicable regulations.

Qualifications

Required

Graduate of an accredited School of Professional Nursing.

Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.

Minimum of one (1) year of nursing experience.

Current Basic Life Support (BLS) certification.

Valid driver's license and insured vehicle in good working condition.

Strong verbal and written communication skills in English.

Ability to pass Pennsylvania State and Federal Criminal Background Checks and Child Abuse Clearances.

Ability to work independently while effectively collaborating with the healthcare team.

Working Conditions

Provide nursing services in patients' homes.

Meet documentation and visit deadlines.

Frequent standing, walking, sitting, bending, stooping, and other physical activities associated with home health care.

Ability to assist with lifting patients weighing over 50 pounds and moving home equipment or furniture when necessary.

Regular travel to patient homes and occasional travel to meetings, conferences, seminars, and training programs.

Ability to communicate effectively in person, by phone, and through electronic documentation systems.
