Position Summary Administers nursing care to patients requiring intermittent nursing service in accordance with physician's orders and under the direction and supervision of a Registered Nurse. Essential Duties and Responsibilities The essential functions include, but are not limited to the following: Complete daily assignments with all progress notes turned in on time Assist Registered Nurse in coordination of total patient care through communication with all disciplines providing care Provide skilled nursing care as outlined in the patients care plan and assigned by the Registered Nurse on the case Observe patient and report changes in condition along with reactions to treatment and report to Director of Clinical Services and/or Registered Nurse on case Document all observations and treatments in clinical notes after each visit Provide personal care as needed and assigned by the Registered Nurse on the case Attend patient team conferences and staff development meetings Recognize your role as part of the QAPI efforts of your organization Attend trainings to build understanding and capacity to undertake QAPI work Carry out QAPI roles and responsibilities as assigned Follow established policies and procedures in support of QAPI efforts Look for and share with leadership ideas for improvement in the organization Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings Minimum Qualifications (Knowledge, Skills, and Abilities) Currently licensed in the state of Pennsylvania At least one (1) year, preferably two (2) years of experience in nursing. On-Call Rotation:
Participation in the hospice on-call schedule is required and includes additional compensation for both call coverage and patient visits during those times.
