Job Summary:
Grace Woods Personal Care Home is a newly renovated personal care community committed to providing compassionate, resident-centered care in a warm, home-like environment. We are growing our team—and this is an exciting time to join Grace Woods! We are looking for a dependable, compassionate, and professional Full-Time LPN who is ready to be part of a growing community. The ideal candidate is flexible, works well independently and as part of a team, and understands that as we grow, responsibilities and processes may evolve.
POSITION OVERVIEW
The LPN is responsible for providing safe, compassionate, and resident-centered care while supporting the overall health and well-being of our residents. This position combines clinical nursing responsibilities with hands-on resident care and universal team responsibilities. The LPN will work closely with residents, families, Resident Care Attendants, Med Techs, administration, physicians, pharmacies, and other professional partners. Because Grace Woods is a growing community, flexibility is essential. We are looking for someone who is comfortable adapting to changes, taking initiative, and helping us build a strong team and culture as we grow.
KEY RESPONSIBILITIES
Resident Care & Nursing Responsibilities
  • Provide compassionate, resident-centered care while promoting dignity, independence, and quality of life.
  • Assist residents with Activities of Daily Living (ADLs) as needed, including bathing, dressing, grooming, toileting, mobility, and other personal care needs.
  • Administer medications safely and accurately in accordance with physician orders, facility policies, and applicable Pennsylvania regulations.
  • Monitor residents for changes in condition and respond appropriately to health and safety concerns.
  • Conduct resident assessments and identify changes in health status and healthcare needs.
  • Develop, implement, and update individualized care plans based on resident needs.
  • Communicate changes in resident condition to appropriate healthcare providers, families, and administration.
  • Complete accurate and timely clinical documentation.
  • Coordinate with physicians, pharmacies, hospitals, hospice, home health, and other healthcare professionals as appropriate.
  • Participate in resident care conferences and ongoing evaluation of resident needs.
  • Follow all applicable Pennsylvania regulations, facility policies, and standards of care. Documentation & Technology
  • Complete required documentation accurately and timely.
  • Utilize PointClickCare (PCC) for electronic health records, documentation, care planning, and other assigned tasks.
  • Maintain accurate resident records and ensure information is communicated appropriately to the care team. Team & Leadership
  • Work collaboratively with Resident Care Attendants, Med Techs, administration, and other team members, and under the direction of the Resident Care Coordinator and Administrator.
  • Provide direction, support, and professional guidance to team members when appropriate.
  • Help maintain a positive, respectful, and team-oriented work environment.
  • As Grace Woods continues to grow, this position may have future supervisory responsibilities for Resident Care Attendants and/or Med Techs.
  • Be willing to take initiative and assist with developing and improving processes as the community grows.
Universal Overnight Responsibilities At Grace Woods, we believe everyone on the team contributes to the overall operation of our home. The overnight LPN is a universal team member and, in addition to nursing responsibilities, may be responsible for assigned overnight operational duties, including:
  • Laundry
  • Cleaning and maintaining common areas and resident areas as assigned
  • Maintaining a clean, safe, and welcoming environment
  • Assisting with other reasonable duties necessary to support residents and the community These responsibilities are part of our team approach and may evolve as Grace Woods continues to grow.
QUALIFICATIONS
  • Current and unrestricted Pennsylvania LPN license .
  • Current CPR and First Aid certification .
  • Previous experience in personal care, assisted living, long-term care, senior care, or a related setting preferred.
  • Strong understanding of resident-centered care and the ability to recognize changes in resident condition.
  • Comfortable assisting residents with ADLs and providing hands-on care.
  • Strong written and verbal communication skills.
  • Professional and compassionate communication with residents, families, physicians, pharmacies, and other professional partners.
  • Comfortable using electronic documentation systems; PointClickCare experience preferred.
  • Ability to work independently and make sound decisions within the scope of the LPN role.
  • Strong organizational and time-management skills.
  • Dependable, responsible, and able to prioritize multiple responsibilities.
  • Ability to work as part of a team and contribute positively to the community.
  • Flexible and comfortable working in a growing environment where responsibilities, processes, and priorities may change.
  • Willingness to assist with universal team responsibilities, including overnight laundry and cleaning duties.
