• 2026 Responsibilities Utilize the nursing process to assess, plan, implement, and evaluate patient care Provide skilled nursing services in accordance with physician orders Assess signs and symptoms indicating changes in patient condition Collect, analyze, and document clinical data and nursing interventions Communicate patient status changes to physicians and care team members Provide patient and family education as appropriate Qualifications Current LPN/LVN license in the state of practice Current TB test or Chest X-Ray Current BLS certification One year of professional nursing experience preferred
