3pm-11pm; every other weekend

Overall Job Purpose The Wellness Nurse provides person-centered nursing care in accordance with Sunshine Retirement Living standards and state regulations. This role focuses on resident health monitoring, early identification of changes in condition, medication management, and compassionate communication with families and healthcare providers. Accurate documentation and collaboration with the care team are essential components of this role. Minimum Job Qualifications and Requirements Current, valid PA LPN license required Experience in long-term care, geriatric, or acute care nursing preferred Prior supervisory experience preferred Ability to communicate clearly and professionally with residents, families, staff, and providers Strong organizational, documentation, and time management skills Ability to work effectively under deadlines and changing priorities Ability to read, write, and interpret clinical and regulatory documentation Team-oriented mindset with a respectful and positive approach Must pass required criminal background check, health screening, and TB clearance as applicable Essential Job Duties and Responsibilities Implement and evaluate resident care plans Administer medications and treatments within scope of licensure Receive, transcribe, and carry out physician orders Monitor resident conditions and promptly document and report changes Ensure resident safety and timely response to resident or family concerns Communicate resident needs and expectations during shift handoff Direct and support Caregivers to ensure care is delivered according to policy and standards Conduct rounds to ensure units are safe, orderly, and resident-focused Maintain accurate clinical documentation and medication records Deliver a high standard of customer service to residents and families Perform additional duties as assigned Additional Responsibilities Promote resident rights, dignity, and safety at all times Coach, mentor, and support care staff while keeping leadership informed of concerns Assist with incident reporting, follow-up, and corrective action when needed Maintain confidentiality of resident and community information Participate in meetings, in-service training, and clinical audits Maintain knowledge of emergency procedures and safety protocols Assist with medication carts, chart audits, and emergency response Maintain required certifications such as First Aid as required by state regulations Serve as a positive role model through professionalism, dependability, and teamwork Adhere to company policies, procedures, dress code, and attendance expectations Make a Meaningful Impact At The Haven at North Hills Memory Care, nursing is more than clinical care.
