Job Description
Join Interim HealthCare as an LPN delivering compassionate home health services in the Healthcare & Medical Services department. Provide direct patient care, medication administration, wound care, vital signs monitoring, and disease management education in clients' homes. Collaborate with RNs, therapists, and families to implement individualized care plans that promote safety, comfort, and independence. Responsibilities Provide direct in-home nursing care under RN supervision Administer medications and treatments accurately Monitor and document vital signs and patient status Perform wound care, dressing changes, and basic procedures Educate patients and families on disease management and home care Follow individualized care plans and report changes to RN/physician Maintain accurate electronic health records and documentation Ensure infection control and patient safety in the home setting Coordinate with RNs, therapists, and caregivers for continuity of care Participate in case conferences and ongoing clinical training Required Skills Medication administration Wound care management Vital signs monitoring Care plan implementation Patient and family education Home health documentation (EMR) Infection control practices Catheter and ostomy care IV therapy and injections Coordination with interdisciplinary team
