Position Summary:

We are seeking a reliable and caring Licensed Practical Nurse (LPN) to join our nursing team. The LPN will provide direct resident care under the supervision of a Registered Nurse (RN) and in accordance with facility policies, state, and federal regulations.

Responsibilities:

Provide nursing care and administer medications as prescribed. Monitor and record residents' vital signs, progress, and changes in condition. Assist physicians and RNs in treatments and care planning. Ensure accurate documentation in electronic medical records. Respond promptly to resident needs and emergencies. Maintain infection control and safety standards. Communicate effectively with residents, families, and the interdisciplinary team.

Qualifications:

Current and valid LPN license in the state. Previous experience in long-term care or rehabilitation setting preferred (but not required). Strong clinical assessment and communication skills. Compassionate, dependable, and team-oriented.
