Description Local home health agency is seeking an experienced LPN to provide one-on-one services to patients in their homes. Duties would include providing medical services to patients. Completing in home assessments and intakes. Assist with admissions. Complete 90 day follow-up care. Ensure the patient care plan is being followed. Complete required notes and documentation. Educate patients and families on care, treatments and answer questions. Employer has full and part time openings. Must be a licensed LPN. Experience in a home health care setting is required.
