Key Responsibilities:
Deliver skilled nursing care to patients in their homes Administer medications, treatments, and wound care Monitor patient conditions and report changes to supervising RN or physician Educate patients and families on care plans and health management Maintain accurate and timely documentation in EMR systems
Qualifications:
Active LPN license in Pennsylvania Minimum of 1 year clinical experience (home health preferred) Strong communication and critical thinking skills Reliable transportation and willingness to travel locally CPR certification required Willing to do on call rotation during the weekends Why Join Us?
