Job Description

Licensed Practical Nurse (LPN) - Day Shift Silver Stream Rehabilitation and Nursing Center 7:00 AM - 7:00 PM | 3 Days Per Week | Every Other Weekend Silver Stream Rehabilitation and Nursing Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team. This position offers a consistent schedule of three 12-hour day shifts per week (7:00 AM-7:00 PM) , including every other weekend , providing an excellent work-life balance while making a meaningful difference in the lives of our residents.
Responsibilities:
Administer medications and treatments as prescribed Monitor residents' conditions and report changes to the RN Supervisor and physicians Provide direct nursing care in accordance with facility policies and standards of practice Accurately document resident care and services provided Supervise and support Certified Nursing Assistants (CNAs) Participate in care planning and interdisciplinary team meetings Ensure compliance with all state, federal, and facility regulations
Qualifications:
Current LPN license in the Commonwealth of Pennsylvania Long-term care experience preferred, but not required Strong clinical, communication, and organizational skills Ability to work effectively as part of a team Commitment to providing exceptional resident-centered care
