Bilingual Care Management Nurse (LPN/LVN) - CCM | PCM | RPM Part-Time | Remote Humble, TX 77338 $18.00-$23.00 per hour Join a Team That Values Compassion, Experience, and Patient Care Ascent Care Partners is seeking an experienced Bilingual Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our Care Management team. We are looking for a compassionate, organized nurse who enjoys building relationships with patients, providing education, and helping individuals successfully manage chronic health conditions. Healthcare experience is required, and previous experience in care management, chronic disease management, primary care, or telephonic nursing is strongly preferred. This is a hybrid position that allows you to use your clinical judgment, communication skills, and nursing experience to improve the lives of patients between office visits. Our nurses support patients through Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM). Rather than providing episodic care, you'll become a trusted resource for your patients, helping them understand their health conditions, manage medications, coordinate care, and stay connected with their healthcare providers. This position is remote. Schedule Monday-Thursday 12:00 PM-6:00 PM Central Time Approximately 10-20 hours per week. Additional hours may become available based on business needs and program growth. Compensation $18.00-$23.00 per hour Compensation is based on experience, qualifications, nursing experience, and relevant care management experience. Responsibilities As a Care Management Nurse, you will: Conduct scheduled CCM, PCM, and RPM patient care calls Develop and update individualized care plans Complete medication reconciliation Educate patients regarding chronic disease management and preventive care Encourage medication adherence and healthy lifestyle choices Identify barriers to care and assist patients in overcoming them Coordinate care with providers, specialists, caregivers, and interdisciplinary healthcare teams Participate in patient outreach and enrollment initiatives, educating patients about available care management services and their potential benefits and helping eligible patients understand available care management services while assisting those who choose to enroll Escalate clinical concerns appropriately Document all patient encounters accurately within the Electronic Medical Record (EMR) Maintain Medicare documentation standards and HIPAA compliance Meet established productivity, quality, and program participation goals We're Looking For Nurses Who Are Compassionate and patient-focused Excellent communicators
