Overture is hiring for a full-time, hybrid Health Services Coordinator in Longmont.
Minimum Requirements:
  • LPN certification/licensure
  • Must be able and willing to work some hours outside of normal working hours to respond to emergencies or to otherwise meet the needs of the people we serve and their families.
  • Must have consistent access to reliable transportation that will allow for quick response in an emergency.
  • Must possess a valid Colorado Driver's License and proof of insurance per Overture insurance guidelines.
  • Must pass background check and other screening per Overture requirements.
Salary Range:
$63,500.00

  • Effectively coordinate necessary medical care, required appointments and maintain documentation of health services according to state regulations for the individuals we serve.
  • Provide consultation to the individuals we serve, host home providers, and Overture program coordinators regarding health care issues related to each individual in services.
  • Provide support and training for managing medication administration and monitoring for side effects. Monitor MARS monthly for proper documentation.
  • Perform self-medication assessments with continued supervision and monitoring as needed and at minimum, quarterly.
  • Coordinate physical, dental, visual, neurological, audiology and other medical appointments for individuals we serve according to state regulations.
  • Work with the Residential and Day Services team to problem-solve and carry out health support actions.
  • Develop crisis prevention strategies and strategies for the management of medical crises as they occur.
  • Interface with Medicaid or Medicare programs as needed.
  • Conduct QMAP classes, G-tube classes and Nursing Orientation classes as directed.
  • Conduct home visits as assigned.

  • This position is a hybrid/flex remote schedule out of your nearest located office and requires travel to our other Colorado locations as needed.
