Licensed Practical Nurse (LPN) Care Coordinator Atlanta Structured Family Services Atlanta Structured Family Services is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to support our care management team in serving elderly and disabled clients in a home-based setting. This role works under the supervision of a Registered Nurse (RN) and plays a key part in supporting caregivers and ensuring quality care delivery. Position Overview As an LPN Care Coordinator, you will assist the RN Care Manager in monitoring client health, supporting caregivers, and ensuring care plans are followed. You will serve as a consistent point of contact for caregivers, providing guidance, reinforcement of care plans, and reporting changes in client condition to the supervising RN. Key Responsibilities Assist with in-home visits to support client and caregiver needs Monitor client condition and report changes to the supervising RN Reinforce care plans and provide ongoing support to caregivers Assist with caregiver training and education under RN direction Review and monitor caregiver documentation and daily task logs Maintain regular communication with caregivers and care team Support completion of required reports and documentation Assist caregivers with program requirements and mobile applications Ensure compliance with program guidelines and standards Perform other duties as assigned Qualifications Active Licensed Practical Nurse (LPN) license in the state of Georgia required Minimum 1-2 years of experience in home health, long-term care, or similar setting Experience working with elderly and/or individuals with disabilities preferred Strong communication and organizational skills Ability to work independently while following RN supervision Compassionate, reliable, and team-oriented Schedule & Work Environment Flexible hybrid role (remote + in-home visits) Travel required within assigned service area Works under the supervision of an
