Job:
Clinical Phone Triage LPN:
Advanced Specialty Care, P.C. is seeking a Full-time Licensed Practical Nurse to support our clinical phone triage team in a multi-specialty setting. This role is primarily focused on clinical phone triage, patient care coordination, and team support while working closely with providers and staff to ensure timely, efficient, and high-quality patient care delivery.

Must be able to work in a fast-paced environment. Ability to work independently while collaborating with providers and clinical staff. Strong clinical judgment and critical thinking skills. Excellent oral and written communication skills. Detail-oriented, organized, and able to prioritize tasks effectively. Ability to multi-task and manage high call/message volumes. Proficient in the use of computers, EMR systems, and software programs.
Responsibilities and Duties:
Performs clinical phone triage by assessing patient symptoms and determining urgency using established protocols. Serves as team lead for triage staff, supporting workflow management, training, and daily operations. Communicates patient concerns to providers and facilitates appropriate follow-up care. Documents all patient interactions, triage decisions, and clinical information accurately in the EHR. Manages incoming calls, patient portal messages, and task workflows within the EMR system. Processes prescription refill requests in accordance with provider direction and practice protocols. Provides patient education and guidance within LPN scope of practice. Coordinates care, including scheduling urgent appointments and follow-up visits as needed. Maintaining accurate and timely clinical documentation in the EHR. Adheres to all clinical protocols, safety standards, and regulatory requirements.
Requirements:
Active CT LPN State license. Active BLS Certification. Minimum 5 years' experience as an LPN, preferably in a clinical triage or outpatient setting. Prior clinical phone triage experience required. Experience with EMR systems required. Strong understanding of clinical workflows and patient communication.
Schedule:
Full Time:
40 hours per week, Monday through Friday.
On-site schedule:
Monday, Tuesday, Thursday (Danbury office).
Remote schedule:
Wednesday and Friday Must be able to work a flexible work schedule.
Training :
3-month initial training period conducted on-site at our New Milford office. Upon completion of training, position transitions to Danbury-based hybrid schedule.
Bonus offered:
