Header Job Summary Join our dynamic home healthcare team as a Licensed Practical Nurse (LPN), where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate, efficient nursing services across various settings. Your skills will support patient assessments, caregiver assessments while ensuring compliance with healthcare regulations and standards. Duties Perform comprehensive patient assessments and health history documentation using electronic health record (EHR) systems. Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or memory care requirements. Educate patients and their families on health coaching topics like medication management, nutrition, discharge planning, and chronic disease management. Maintain accurate medical documentation adhering to HIPAA regulations. Collaborate with multidisciplinary teams in inpatient, outpatient, hospice, or home & community care environments to ensure seamless patient care transitions. Proven experience working in diverse healthcare environments including hospitals (ICU, ER), nursing homes, outpatient clinics, or home care. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment. Experience caring for individuals with developmental disabilities or those requiring specialized dementia or memory care. Excellent communication skills. Completing competency assessments on CNA's and PCA's Familiarity with infection control protocols and sterile processing techniques.
