Job Description

Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and coordinated care. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and sterile techniques. Assist with complex procedures such as IV infusion, catheterization, tube feeding, and wound suturing to ensure optimal patient outcomes. Monitor patients' physiological status during acute care episodes in settings like ICU or emergency rooms, providing critical support for airway management and ventilator use. Conduct triage assessments for urgent care or emergency room visits to prioritize patient needs effectively. Collaborate with multidisciplinary teams including physicians, case managers, and specialists to develop comprehensive discharge plans and coordinate ongoing care. Maintain detailed medical documentation of patient care activities, medication administration, and diagnostic evaluations to ensure compliance with healthcare standards. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, long-term care facilities, or outpatient clinics. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation and patient record management. Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/ICD-10). Experience with specialized care areas including Alzheimer's/dementia care, memory care, hospice support, pediatric or neonatal care preferred. Competence in performing medical procedures such as phlebotomy, venipuncture, sterile processing techniques, and basic life support (BLS). Ability to work effectively with individuals with disabilities or developmental challenges while demonstrating empathy and professionalism. $30.00 - $45.00 per hour
