Job Description

Sign-On Bonus Available Job Summary Assumes responsibility for daily coordination of the resident's care, including comprehensive assessments of physical and mental condition. The RN-MDS Coordinator oversees, coordinates, & assumes responsibility for resident care which includes but is not limited to admission, discharge, and transitions of care. The MDS Coordinator assures that quarterly, annual, & significant change observations & assessments are completed in a timely fashion for assigned group of Residents. Member acts as a resident advocate, provides leadership, organization, and direction to the nursing department. Essential Job Functions Makes nursing assessments regarding the health status of the resident and is knowledgeable of the resident's condition at all times and reports and documents pertinent changes to family, physicians, staff and Director of Nursing. Audits resident charts, care plans, and charting to ensure documentation supports the care provided and reimbursement level. Maintains clear and consistent communication with the Director of Nursing to ensure timely awareness of resident care documentation, compliance issues, and staff performance related to MDS coding and assessments. Completes and transmits MDS assessment and reports in accordance with regulatory requirements and facility policies and procedures. Utilize the MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs) Oversees the documentation and conducts interviews with residents and staff to ensure alignment and accuracy with MDS coding requirements. Collecting, documenting, and submitting MDS according to State and Federal guidelines/ timelines Identifies and communicates need for significant change in status assessments. Ensures compliance with guidelines for participation in Medicare, Medicaid and other benefit programs. Maintains a viable working relationship with all disciplines involved in the planning of resident care. Completes Discharge Assessment and entry/re-entry tracking forms per Federal regulations. Schedules MDS assessments per guidelines, as assigned and maintains a monthly calendar. Notifies members of the IDT of changes or additions to monthly Care Plan Meeting schedule, for new admission, change of condition or interim meetings. Electronically submits MDS information. Reviews, revises, corrects, and resubmits as appropriate;. Contributes to the Interdisciplinary care plan by communicating any resident issues that are pertinent to the plan of care. Provides all requested information for annual MDS reviews Required to work every third weekend and participate in a holiday rotation schedule, including on-call responsibilities during assigned weekends. Participates in interdepartmental meetings as needed and attends when indicated or requested Other duties as assigned
