Standing Stone Care and Rehab Center in Cookeville, TN is seeking a compassionate Licensed Practical Nurse to provide resident-centered care in a skilled nursing and rehab setting. The LPN will administer medications and treatments, monitor resident conditions, support rehab plans, and document care accurately. Collaborating with RNs, therapists, and physicians, you will help develop and follow individualized care plans while educating residents and families. Responsibilities Provide direct nursing care to residents in skilled nursing and rehab settings Administer medications and treatments per physician orders and facility policy Monitor, document, and report changes in resident condition to the RN/physician Support and implement individualized care and rehab plans Accurately complete electronic health records and required documentation Assist with ADLs, wound care, and restorative nursing interventions Educate residents and families on care plans, medications, and health needs Collaborate with RNs, therapists, physicians, and CNAs to coordinate care Follow infection control, safety, and regulatory guidelines Participate in quality improvement and resident-centered initiatives Required Skills Medication administration Wound care Vital signs monitoring Electronic health records (EHR) documentation IV therapy (per state scope and training) Infection control practices Patient assessment and observation Rehab and restorative nursing techniques Care planning support Patient and family education
