Charter Senior Living of Hermitage is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our care team. This role is ideal for a nurse who is passionate about providing quality care to seniors while working in a supportive, team-focused environment. As an LPN at Charter Senior Living, you will play an important role in supporting the health, comfort, and dignity of residents in our Assisted Living and Memory Care programs.

POSITION SUMMARY

The Licensed Practical Nurse (LPN) is a key member of the care team responsible for supporting the health and wellbeing of residents. This position assists residents through clinical assessments, care planning, medication and treatment management, and documentation of nursing services. The LPN helps ensure residents receive quality care while maintaining their dignity and independence.

KEY RESPONSIBILITIES

Provide nursing care and clinical support to residents in Assisted Living and Memory Care Administer medications and treatments according to physician orders Assist with resident assessments and care planning Monitor resident health and report changes in condition Maintain accurate documentation of nursing services Collaborate with caregivers and leadership to ensure quality care Promote resident comfort, dignity, and quality of life

QUALIFICATIONS

Active Licensed Practical Nurse (LPN) license required Previous senior living or assisted living experience preferred Strong clinical and observation skills Excellent communication and teamwork abilities Compassionate approach to resident care Ability to thrive in a collaborative care environment
