Coordinator - Full-Time Smoky Mountain Health & Rehabilitation Center 415 Cole Drive, Pigeon Forge, TN 37863 Full-Time Smoky Mountain Health & Rehabilitation Center is seeking an experienced RN or LPN MDS Coordinator to join our team in Pigeon Forge, TN. Position Overview The
RN/LPN MDS
Coordinator is responsible for coordinating and overseeing the Resident Assessment Instrument (RAI) process , ensuring the accuracy, completeness, and timely completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs), and interdisciplinary care plans . This position works closely with nursing leadership and the interdisciplinary team to promote accurate resident assessments, appropriate care planning, regulatory compliance, and reimbursement. MDS Coordinator Responsibilities Coordinate and oversee the completion of MDS assessments accurately and within required timeframes Monitor and guide the interdisciplinary team throughout the RAI/MDS process Review MDS documentation for accuracy, completeness, and regulatory compliance Oversee the submission and transmission of MDS assessments and obtain validation of successful submissions Coordinate and manage the resident care planning process Schedule and facilitate interdisciplinary care plan conferences Ensure resident care plans accurately reflect each resident's current needs, goals, and services Facilitate monthly Quality Measure (QM) meetings Monitor quality measures and identify opportunities for improvement Maintain compliance with federal, state, and local regulations and guidelines Collaborate with nursing, therapy, dietary, social services, and other members of the interdisciplinary team Support accurate reimbursement through proper assessment, documentation, and coding Qualifications Current, active, and unencumbered Tennessee RN or LPN license Nursing degree or diploma from an accredited nursing program Current CPR certification Minimum of 2 years of Long-Term Care MDS Coordinator experience Strong knowledge and experience with the MDS/RAI process and care planning RAC-CT certification preferred Experience with Medicare, Medicaid, reimbursement, and Case-Mix preferred Skills & Experience The ideal candidate will have: Strong knowledge of MDS, RAI, Case-Mix, Medicare reimbursement, and Medicaid reimbursement Knowledge of current federal, state, and local regulations governing skilled nursing and long-term care facilities Excellent written and verbal communication skills Strong critical-thinking and problem-solving abilities Excellent organizational and time-management skills Ability to prioritize responsibilities and meet required deadlines Strong attention to detail and documentation accuracy Ability to effectively collaborate with an interdisciplinary healthcare team Why Join Smoky Mountain Health & Rehabilitation Center?
