Travel Assignment Schedule:
Monday•Friday, 8:00 AM•4:30 PM (40 hours/week) Key Responsibilities Perform skilled nursing visits in patients' homes. Provide care in accordance with physician orders and established plans of care.
Deliver specialized nursing services including:
Wound VAC managementPICC line care and maintenancePleurX drain careWound care treatments Other skilled nursing interventions as needed Monitor patient conditions and report significant changes to the appropriate clinical team members. Educate patients and caregivers on treatments, medications, and disease management. Complete timely and accurate clinical documentation. Collaborate with physicians, RNs, therapists, and other healthcare professionals to ensure continuity of care. Participate in on-call and weekend coverage as required. Required Qualifications Current and active LVN/LPN License Current BLS Certification Previous Home Health experience Preferred Qualifications Experience with Homecare Homebase (HCHB) Strong wound care and infusion-related skills Ability to work independently in a field-based environment Excellent communication and patient education abilities Required Certifications LVN/LPN License BLS Certification
