Licensed Vocational/Practical Nurse Job Summary:
Primary function is to administer skilled nursing care, under the supervision of a registered nurse, for patients of all ages in their places of residence. Participates in the coordination of care with the interdisciplinary team, patient, family, and referring agency. We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) to join our dedicated healthcare team. In this vital role, you will provide high-quality, personalized nursing care directly in patients' homes and community settings. Your energetic approach will ensure patients receive comprehensive assessments, medication management, and ongoing support to promote health, safety, and independence. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals across diverse care environments. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews to develop individualized care plans. Administer medications, injections, IV infusions, and treatments such as wound care or catheterization following strict protocols and safety standards. Perform specialized procedures like tube feeding, sterile dressing changes, venipuncture, and medication administration with precision and compassion. Document all patient interactions accurately using Electronic Health Record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Collaborate with physicians, case managers, and other healthcare professionals to coordinate care plans for patients with complex needs including dementia, Alzheimer's disease, or chronic conditions. Provide education on disease management, medication adherence, nutrition, and lifestyle modifications tailored to individual needs. Assist with discharge planning, home safety evaluations, and referrals for additional services like hospice or post-acute care when necessary.
Job Qualifications:
Education:
Graduate of an accredited school of vocational/practical nursing.
Licensure:
Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state. Current driver's license.
Experience:
One year of experience as a licensed vocational/practical nurse in a clinical care setting required. Home health experience, preferred.
Skills :
Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR certification.
Transportation:
Reliable transportation and valid auto liability insurance. Excellent communication skills for triage calls, patient education sessions, and interdisciplinary team collaboration in fast-paced environments such as urgent care or hospital medicine.
