In this vital role, you will provide high-quality nursing care to diverse patient populations, including geriatric, pediatric, and individuals with developmental disabilities. Your expertise will support patient recovery, comfort, and overall well-being through comprehensive assessments, medication administration, and personalized care plans. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform care strategies. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Manage IV infusions and catheter care, ensuring sterile techniques and patient safety at all times. Assist with complex procedures such as wound suturing, tube feedings, and airway management. Document all patient interactions meticulously within Electronic Health Record (EHR) systems like Epic or Cerner, maintaining compliance with HIPAA regulations. Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments in memory or assisted living facilities. Collaborate with interdisciplinary teams to develop and implement personalized care plans for acute and chronic conditions including pain management, infectious diseases, and post-operative recovery. Qualifications Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience working with diverse patient populations in settings such as long-term care facilities, hospitals (including ICU or trauma centers), hospice, or home health environments. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Hands-on experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth; proficiency in medical documentation is essential. Skilled in medication administration, venipuncture (phlebotomy), IV insertion, and sterile technique practices. Familiarity with ICD coding (ICD-9/10), CPT coding for procedures, and long-term care regulations is highly desirable. Ability to perform physical examinations and assessments accurately while demonstrating excellent patient care skills. Experience working with individuals with disabilities or behavioral health needs; knowledge of dementia or Alzheimer's care is a plus.
