We are seeking an experienced and compassionate Home Infusion Nurse ( LVN) who values clinical excellence, accountability, and meaningful patient care. We are looking for someone who can work full-time. This role is ideal for a nurse who thrives in an autonomous environment while remaining closely connected to a collaborative healthcare team. Position Overview The Home Infusion LVN is responsible for delivering skilled nursing care in the home setting, including the administration of intravenous therapies, patient assessment, and education. This position requires strong clinical judgment, attention to detail, and the ability to build trust with patients and caregivers. Key Responsibilities Administer IV therapies, including antibiotics, and parenteral nutrition medications in the home setting Perform thorough patient assessments before, during, and after infusion therapy Access and maintain vascular access devices, including PICC lines, ports, and peripheral IVs Program and manage infusion pumps safely and accurately Monitor patients for adverse reactions and respond in accordance with clinical protocols Educate patients and caregivers on infusion therapy, line care, infection prevention, and medication management Support patient independence through self-administration training when appropriate Collaborate with physicians, pharmacists, and internal care teams to ensure continuity of care Maintain accurate, timely documentation in compliance with regulatory standards and company policies Ensure proper handling, storage, and disposal of medications and supplies Adhere to strict aseptic technique and infection control standards Participate in on-call rotation and ongoing training to maintain clinical competency Qualifications Active Nursing license (LVN) in Texas 1-3 years of clinical nursing experience (infusion, ICU, ER, or home health preferred) New grads are considered. Demonstrated competency in IV therapy and central line care Current BLS certification (ACLS preferred) Strong clinical assessment and critical thinking skills Excellent communication and patient education abilities Ability to work independently while maintaining strong collaboration with the care team Valid Texas driver's license and reliable transportation Work Environment & Expectations Provide care in patient homes across assigned service areas Flexible scheduling, including afternoons, evenings, and weekends as needed Adherence to all Landmark Healthcare policies, regulatory requirements, and accreditation standards Why Landmark Healthcare At Landmark Healthcare, we believe that exceptional care starts with exceptional clinicians.
