About CareTrends CareTrends, Inc. is a home health agency dedicated to providing quality, compassionate care to patients in the comfort of their own homes. We are seeking dependable, compassionate, and professional LVNs to join our growing team. Position Summary The Licensed Vocational Nurse (LVN) provides skilled nursing care to patients in their homes according to the patient's individualized plan of care and physician/provider orders. This is a field-based position serving patients throughout the DFW area. The ideal candidate is dependable, compassionate, comfortable working independently, and able to communicate effectively with patients, families, caregivers, and the CareTrends team. Responsibilities Provide skilled nursing care according to the patient's plan of care and physician/provider orders. Monitor patients' conditions and report changes appropriately. Administer medications and treatments as ordered. Perform nursing assessments and skilled nursing interventions within the scope of practice. Provide wound care and dressing changes in accordance with the patient's plan of care and physician/provider orders. Monitor wounds for signs of infection, changes in condition, and healing progress, and report concerns appropriately. Document wound assessments, treatments, and patient response accurately and timely. Provide patient and family education regarding medications, treatments, and care plans. Communicate effectively with patients, families, caregivers, physicians/providers, and the CareTrends team. Follow agency policies, care plans, physician/provider orders, and applicable regulations. Promote patient safety and quality of care. Maintain patient confidentiality. Complete all required clinical and visit documentation accurately and on time. Attend required training and staff meetings. Perform other nursing duties as assigned. Qualifications Current, active, and unrestricted Texas LVN/LPN license . Current CPR/BLS certification, as required. Previous nursing experience preferred. Home health experience preferred. Wound care experience preferred. Strong communication and interpersonal skills. Ability to work independently in a home setting. Strong organizational and documentation skills. Reliable transportation and ability to travel throughout the DFW area.
