Accurately complete all required documentation, charting, and reports in accordance with regulatory requirements and facility policies. Support and implement individualized resident care plans while ensuring resident dignity, preferences, and rights are respected. Observe residents closely and report significant changes in physical, mental, or emotional condition to the interdisciplinary care team Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care
