Job Description

Job Overview We are seeking a dynamic and compassionate Licensed Vocational Nurse (LVN) to join our dedicated healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise in nursing practices, patient assessment, and medical documentation will ensure that our patients receive personalized and effective care. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse healthcare needs while working within a supportive and energetic environment. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using EMR (Electronic Medical Records) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusion and medication administration techniques. Assist with complex procedures such as catheterization, wound suturing, and sterile processing to support patient recovery and comfort. Provide specialized care for individuals with dementia, Alzheimer's disease, memory care needs, and behavioral health conditions. Support acute pain management, hospice care, trauma response in Level I and Level II trauma centers, as well as emergency medicine scenarios including airway management and ventilator support. Conduct patient triage in urgent care or emergency room settings to prioritize care based on severity of condition. Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care to ensure seamless patient transitions across settings. Qualifications Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience in critical care environments such as ICU or trauma centers is highly desirable. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Experience working with diverse populations including pediatrics, geriatrics, neonatal ventilator management, and individuals with disabilities or developmental delays. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation. Ability to perform phlebotomy, venipuncture, tube feeding management, and basic life support (BLS). Excellent patient assessment skills combined with compassionate communication and teamwork abilities.
