This role involves providing comprehensive patient care in a home setting, supporting individuals with diverse medical needs, including geriatrics, chronic conditions, and post-acute care. The ideal candidate will possess extensive clinical experience, including familiarity with EMR/EHR systems, medical documentation, and various specialized care areas such as IV infusion, wound care, and medication administration. The LVN will work closely with patients, families, and interdisciplinary teams to deliver high-quality nursing services that promote health, safety, and independence. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation. Administer medications and injections accurately following physician orders while adhering to HIPAA and infection control protocols. Perform specialized procedures such as IV insertion, catheter care, tube feeding management, and sterile dressing changes. Monitor patients' conditions closely, including managing acute pain, respiratory issues (ventilator management), and infection control measures. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Educate patients and families on disease management, medication use, nutrition, and safety measures. Document all patient interactions thoroughly using EMR/EHR systems like Epic, Cerner, or Athenahealth. Coordinate discharge planning and post-acute care services to ensure seamless transitions across settings. Provide support for patients with complex needs such as dementia care, Alzheimer's care, behavioral health issues, or developmental disabilities. Maintain compliance with all healthcare regulations including NCQA standards and OSHA guidelines. Experience Candidates should have a robust background in various clinical settings including: Long-term care facilities such as nursing homes or assisted living communities. Acute care environments like ICU or emergency room experience at Level I or Level II trauma centers. Home & community-based care with experience in post-acute or sub-acute settings. Pediatric or neonatal units (PICU or neonatal ventilator experience) is a plus for working with children or infants. Experience with complex procedures such as spinal taps, dialysis support, or dermal fillers is advantageous. Proficiency in medical coding (ICD-9/10), CPT coding, phlebotomy, specimen collection/processing is preferred. Candidates must demonstrate excellent patient assessment skills along with knowledge of physiology and medical terminology. Prior exposure to EMR/EHR systems like eClinicalWorks or Meditech is highly desirable.
