  • Permanent, Full-time Licensed Nurse Practitioner We are seeking an experienced, independent, highly skilled, and technology-proficient Nurse Practitioner (NP) to join our dynamic healthcare team in Pilot Point.
The ideal candidate will deliver comprehensive primary care services with a strong emphasis on patient-centered preventive medicine while leveraging advanced healthcare technology, including Clinical Decision Support Systems (CDSS), Remote Patient Monitoring (RPM), telehealth services, and AI-powered clinical documentation tools. Training will be provided for all clinical systems and digital tools. In Texas, the NP will practice under a delegation agreement with a supervising physician, ensuring compliance with state regulations while maintaining a high degree of clinical autonomy in day-to-day patient care. Key Responsibilities Provide comprehensive primary care services, including: Preventive care Chronic disease management Acute illness treatment Develop and implement personalized preventive care plans focused on: Lifestyle modification Early disease detection Risk factor management Utilize Clinical Decision Support Systems (CDSS) to enhance. Diagnostic accuracy Evidence-based treatment planning Implement and manage Remote Patient Monitoring (RPM) programs to: Track chronic conditions Enable early intervention Supervise and manage programs to ensure quality care and timely interventions. Remote Patient Monitoring (RPM) Chronic Care Management (CCM) Remote Therapeutic Monitoring (RTM) Conduct and support telehealth consultations to Expand access to care Collaborate with physicians and specialists Leverage AI-powered scribing tools to: Streamline clinical documentation Improve workflow efficiency Collaborate with physicians and multidisciplinary teams to deliver: Integrated, patient-centered care Educate patients on preventive healthcare strategies, including: Nutrition Exercise Vaccinations Routine screenings Ensure compliance with: Health Insurance Portability and Accountability Act (HIPAA) Texas state regulations Organizational policies Stay current with advancements in: AI in healthcare Telemedicine Digital health platforms Participate in quality improvement initiatives to: Enhance patient outcomes Optimize preventive care delivery Qualifications & Skills Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution Board-certified Nurse Practitioner (Family Medicine or related specialty) Active Texas NP license (or eligibility to obtain) Practice under a delegation agreement with a supervising physician BLS required; ACLS preferred Spanish-speaking ability (strong plus) Proficiency with Electronic Health Records (EHR) Familiarity with Clinical Decision Support Systems (CDSS) and AI-assisted documentation tools is preferred, but not required. Experience with Telemedicine and Remote Patient Monitoring (preferred) Strong commitment to: Preventive care Evidence-based medicine Patient-centered outcomes
