Role Overview:

As an LPN - Skilled Home Care at BrightStar Care, you will be responsible for delivering high-quality nursing care in patients' homes. In this role, you'll collaborate closely with families and care teams to ensure the best possible outcomes for each client. Your work will focus on one-on-one patient care, enabling meaningful connections with clients and their families. This is an on-site position in Richardson, Texas, allowing you to provide essential care directly in the community.

Key Responsibilities:

Providing skilled nursing care in the home setting. Administering and managing medications as per prescribed plans. Monitoring and documenting patient conditions regularly. Collaborating effectively with families and multidisciplinary care teams. Assisting with treatments and implementing physician-ordered care plans. Ensuring patient comfort, safety, and overall well-being.

Qualifications:

You're a great fit if you have the following skills. Current Texas LVN license in good standing. Strong clinical skills complemented by excellent communication abilities. A compassionate and dependable attitude towards patient care. Preferably have home health or private duty nursing experience. Current CPR certification is preferred. Reliable transportation to visit patients.

Bonus If You:

Have experience working in a home care setting. Possess additional certifications in specialized care areas.
