Overview Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN), where your expertise will make a meaningful difference in patient care across diverse settings. This vital role involves providing compassionate, skilled nursing services to individuals of all ages, including those with complex medical needs. You will work closely with interdisciplinary teams to deliver high-quality care, ensure patient comfort, and promote health and wellness. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform care plans. Administer medications and injections accurately, including IV infusions and specialized treatments such as botulinum toxin therapy. Manage complex medical procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes using aseptic techniques. Utilize electronic health record (EHR) systems like Epic, Cerner, or Athenahealth to document patient information meticulously and ensure compliance with HIPAA regulations. Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, traumatic injuries (Level I & II trauma), infectious diseases, and chronic pain management. Assist in triage assessments for urgent or emergent cases in emergency rooms or urgent care settings to prioritize patient needs effectively. Support discharge planning and case management activities to facilitate smooth transitions from hospital or long-term care facilities to home or community-based settings. Requirements Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience in diverse healthcare environments including hospitals, nursing homes, assisted living facilities, or home care. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical documentation. Critical care experience preferred, including ICU or trauma center exposure; experience with Level I & II trauma centers is highly valued. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Ability to perform advanced nursing procedures such as IV insertion, phlebotomy, suturing, and ventilator management. Excellent patient assessment skills with a focus on senior care, pediatrics, neonatal ventilator management, or working with individuals with disabilities. Knowledge of infection control practices and adherence to HIPAA standards to ensure patient confidentiality and safety.
