About the
Role:
As an LVN, you will work on a per diem basis, providing direct patient care and supporting our nursing team in delivering high-quality care to our patients. You will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care transitions. Your expertise and compassion will make a significant difference in the lives of our patients and their families.
Responsibilities:
Provide direct patient care, including administering medications, performing treatments, and monitoring patient condition. Collaborate with other healthcare professionals to develop and implement individualized care plans. Conduct patient assessments, monitor vital signs, and report any changes or concerns to the nursing team. Participate in quality improvement initiatives to ensure excellent patient outcomes. Maintain accurate and detailed records of patient care and progress.
