Job Description

Rocky Mountain Care is seeking amazing NURSES to join our team! Tired of answering never-ending call lights? Mandatory call shifts/overtime? Are your shifts constantly being shuffled between days and nights? Come work with us and enjoy scheduling your own visits around your availability. This position is responsible for the delivery of care to a group of residents in a nursing unit, including assessing resident needs, development of individual care plans, administering nursing care, and supervising Certified Nursing Assistants (CNAs) activities and other personnel in the delivery of nursing care. Responsibilities and Duties Receive information from referral sources and coordinate care for patients Verify and enter physician orders Identify level of care needed Receive and give verbal orders accurately Review clinical documentation to determine Skilled Nursing placement Exceptional interpersonal and customer service skills Ability to communicate clearly with pharmacist Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families Manage after-hours and weekend referrals (rotating on call)
Qualifications:
Current license as a licensed LPN from the Utah State Board of Nursing Good verbal and written communication skills Basic computer knowledge Ability to effectively problem solve Provide excellent customer service Knowledge of Medicare/Medicaid home care and hospice criteria preferred Knowledge of Medicare, Medicaid and 10A criteria "Consistent with the Americans with Disabilities Act (ADA) and the applicable federal and state civil rights laws, it is the policy of Rocky Mountain Care, and its affiliates and managed entities (together hereafter referred to as "RMC"), to provide reasonable accommodation when requested by a qualified applicant or employee with a disability, unless such accommodation would cause an undue hardship.
