Job Description

EnCircle helps people with developmental disabilities lead person centered and abundant lives. We have an immediate need for a full-time well-qualified Health Coordinator. The Health Coordinator position is for an LPN to oversee the medical component of group home operations. The LPN Health Coordinator performs tasks related to medical appointments, medication management, skilled community assessment visits, and home medical supports.
Please note:
This role requires driving, therefore a "clean driving record" is critical.

MINIMUM REQUIREMENTS
Must be 18 years of age or older. Current, Valid LPN license.
BLS, CPR.
Valid driver s license and proof of insurance required. Satisfactory Driving Record with no negative points balance. Ability to pass a background check and drug screen. Must have responsible work ethic and good communication skills. Must be compassionate and caring. Availability for home visits in community.
Schedule:
Monday-Friday, Daytime hours. Rotational on-call responsibility.
DRIVING SPECIFIC
 (The following list may or may not be all inclusive at time of posting): Must not have any of the following violations within the last 5 years: Reckless driving, including speeding 20mph or more over the posted speed limit. Driving under the influence/impaired/intoxicated or refusing a blood/breath test. Manslaughter or negligent homicide. Driving after being declared a habitual offender. Driving on a suspended or revoked license Driving a commercial motor vehicle while disqualified, under the influence, with a blood alcohol level of .04 or higher, refusing a blood/breath test, or violating an out-of-service order. Injuring a person while racing
  • felony. Failure to stop at the scene of a crash where there is injury, death, or property damage of $1000 or more. Emergency vehicle violation
  • death. Vehicular assault or willful stopping, impeding, or damaging a vehicle. Blocking access to the service facility. Attempting to elude police. Operating an unsafe vehicle. Vulnerable Road User
  • Death.
Must not have any of the following violations within the last 3 years: Passing a stopped school bus. Careless driving. Three or more minor moving violations within the last three years.
SPECIFIC DUTIES AND RESPONSIBILITIES
Provide skilled nursing care in accordance with physician orders, nursing standards, and agency policies. Monitor medication administration practices in sponsored and group home residential settings. Coordinate medical appointments and follow-up care as needed. Provide health-related training and competency assessments to direct support professionals and sponsored providers. Complete nursing assessments, notes, health summaries, and incident reports follow-up documentation As directed by the Nursing Supervisor, assist with screening assessments for new individuals. Maintain communication with service coordinators or other assigned contacts and day support managers. Keep them informed of any relevant and pertinent issues related to their individuals. Advocate for individuals' healthcare needs and access to services. Participate in nurse on-call rotation as directed by Nursing Supervisor. Perform other related duties as required and assigned.
