We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide one-on-one care for a patient in a home setting. The ideal candidate will have hands-on experience with tracheostomy (trach) care and a strong commitment to delivering high-quality, patient-centered care in a comfortable home environment.

Key Responsibilities:

Provide direct nursing care to a patient with a tracheostomy in the home setting Perform trach care, suctioning, and monitoring of respiratory status Administer medications and treatments as prescribed Monitor vital signs and assess patient condition, reporting changes promptly Maintain accurate and timely documentation of care provided Collaborate with the patient's healthcare team and family members Ensure a safe, clean, and supportive environment for the patient

Qualifications:

Current and active LPN license in Virginia Minimum of 1 year of nursing experience (home health preferred) Demonstrated experience with tracheostomy care and airway management Current CPR certification (BLS required) Strong clinical, communication, and critical thinking skills Ability to work independently and adapt to a home care environment

Preferred Qualifications:

Experience with ventilator care Pediatric or adult home care experience
