Our dedicated team is committed to providing comprehensive healthcare tailored to meet the diverse needs of our community. Overview We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) specializing in family medicine to join our team as a Refill Nurse. This vital role involves managing patient medication refills, supporting clinical workflows, and ensuring high-quality patient care within a busy primary care setting. The ideal candidate will bring a strong background in medical office procedures, patient assessment, and medication management, with experience working with diverse patient populations including seniors and individuals with chronic conditions. Responsibilities Collaborate with physicians and healthcare teams to process medication refill requests efficiently and accurately. Conduct patient assessments, including vital signs and health history updates, to support clinical decision-making. Manage electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth to document patient interactions and medication orders precisely. Assist in triage by evaluating patient symptoms and determining urgency levels for appropriate follow-up or intervention. Provide patient education on medication usage, side effects, and adherence strategies to promote optimal health outcomes. Support clinic operations by preparing exam rooms, handling specimen collection or processing when necessary, and maintaining sterile techniques. Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information at all times. Qualifications Valid LPN license with current registration in the state of practice. Proven experience in medical office settings, preferably within family medicine or primary care clinics. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and order entry. Strong knowledge of medication administration protocols including injections, IV infusion, and catheter care. Experience working with diverse populations including seniors, individuals with developmental disabilities, dementia or Alzheimer's disease care needs. Ability to perform physical examinations, vital signs measurement, and basic diagnostic evaluations accurately. Excellent communication skills with the ability to educate patients effectively about their medications and health plans. Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality at all times.
