48 Hours per Week Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a 13-week contract assignment. The ideal candidate will provide high-quality nursing care to residents in a skilled nursing and rehabilitation setting while collaborating with an interdisciplinary healthcare team to ensure exceptional patient outcomes. Responsibilities Provide direct nursing care to residents in accordance with facility policies and state regulations. Administer medications and treatments as prescribed. Monitor and document patient conditions, vital signs, and responses to treatment. Assist with resident assessments and care planning. Communicate effectively with physicians, nurses, residents, and family members. Respond promptly to changes in resident conditions and emergencies. Maintain accurate and timely clinical documentation. Support infection control and safety protocols.

Beds Current Census:

Approximately 57 Residents Three Resident Care Wings Qualifications Current Licensed Practical Nurse (LPN) license in Vermont or compact state eligibility. Previous experience in long-term care, skilled nursing, rehabilitation, or geriatric nursing preferred. Current BLS/CPR certification. Strong clinical, communication, and documentation skills. Ability to work independently and as part of a multidisciplinary team.
