Job Description

Overview Licensed Practical Nurse (LPN) - Long Term Care travel assignment in Middlebury, VT, United States. Start date 09/17/2026 for a 5-week contract. Shift is 12-hour day shifts, 3x12 schedule. Weekly pay ranges from $1,384 to $1,463. This position is in a post-acute skilled nursing rehab unit within a long-term care setting. The assignment includes travel with assignment-specific credentialing requirements through Healthforce Healthcare LLC.

Credentialing:
VT license required at submission; complex background checks and credentialing per facility policy Responsibilities Provide direct patient care to adults 60+ in a post-acute long-term care setting Administer medications and perform routine nursing tasks within scope of practice Document in the electronic medical record (EPIC) and communicate care plans with the interdisciplinary team Support activities of daily living, safety, and comfort; monitor for changes in condition Perform phlebotomy, EKGs, transports, and other unit procedures as allowed; assist with starting IVs where permitted Collaborate with RNs, CNAs, and other staff to ensure smooth workflow and high-quality care Respond promptly to call lights and participate in unit rounds and care planning Adhere to dementia-capable care standards when applicable and provide patient-family education as needed
Requirements Licensure:
Vermont LPN license required at time of submission
Certifications:
BLS required; IV certification preferred for
LPNs Experience:
Minimum 1 year of clinical experience; prior travel experience required
EMR:
EPIC experience within the past 2 years required
Education:
Standard practical nursing education; appropriate state licensure
Additional:
Dementia-capable care certification preferred; ability to work in a post-acute long-term care unit with geriatric patients
Credentialing:
