As an LPN Licensed Practical Nurse on our team, you'll play a vital role in delivering high-quality care that exceeds our patients' expectations. About the
Role:
We're seeking a compassionate and dedicated LPN to join our dynamic team at St. Johnsbury Center for Living and Rehabilitation, located in St. Johnsbury, VT. As an LPN, you'll work alongside a multidisciplinary team to provide comprehensive care to our patients, including: Assessing and monitoring patients' conditions Developing and implementing individualized care plans Administering medications and treatments Providing wound care and dressing changes Assisting with daily living activities and promoting independence
Responsibilities:
Conduct resident assessments and create individualized care plans Administer medications and treatments as ordered by the physician Assist with daily living activities, including bathing, dressing, and grooming Provide emotional support and education to residents and their families Participate in interdisciplinary team meetings to discuss resident care and progress
