LPN, Wellness Director at The Courtyard at Fitchburg Memory Care

The Courtyard at Fitchburg Memory Care is seeking an experienced LPN Wellness Director to oversee the clinical operations for our memory care residents. The LPN Wellness Director is a key component to providing care and services to the residents in a home-like environment in which they receive individualized services based on their needs and preferences. The Nurse assists with the medical components related to their care and services. Job duties include but not limited to: Assessments Care Planning (Development and Meetings) Coordinating Care and collaboration amongst internal teams, physicians, and third party providers Policy development and implementation Employee education and training Oversight of medication management program Quality assurance Completing medication reviews Resident education Must be a licensed nurse in the State of WI On call for staff to report and triage (rotates on weekends)
