We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team. Responsible for:
Providing direct patient care, including medication administration, wound care, and vital sign monitoring Developing and implementing individualized care plans in collaboration with the interdisciplinary team Maintaining accurate and detailed records of patient care and medication administration Assisting with patient transfers, lifts, and repositioning as needed Participating in shift report and bedside report with the healthcare team Communicating effectively with patients, families, and healthcare team members Maintaining a safe and clean environment, adhering to infection control protocols Participating in quality improvement initiatives and training programs About the
