! PCC No Scrub color Responsibilities for this position include, but are not limited to: Provide direct nursing care to residents in a Long-Term Care (LTC) setting. Administer medications and treatments according to physician orders. Monitor, assess, and document residents' conditions and report changes promptly. Collaborate with physicians and the interdisciplinary care team to develop individualized care plans. Maintain accurate and timely clinical documentation. Ensure compliance with infection prevention, patient safety, and facility policies. Educate residents and families on treatments and healthcare plans. WI LPN/LVN
License. Minimum 1 year of LTC or Skilled Nursing experience preferred. BLS Certification required. Ability to work Evening shifts with occasional Day shifts. Must be available to work Every Other Weekend (EOW). Willing to work 5 x 8-hour shifts per week. Overtime availability is a plus.
