Schedule:
2:00 pm to 10:30 pm, including every other weekend & holiday rotation
Wage:
$26.37 - 31.13 per hour Additional pay: Applicable shift and weekend differentials

$26.37 - 31.13 per hour Additional pay: Applicable shift and weekend differentials Position Summary Lakeview Health Center & Assisted Living is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high quality resident care within our long term care setting. Under the supervision of a Registered Nurse, the LPN delivers direct nursing care, supports care planning, and helps ensure a safe, respectful, and person centered environment. Key Responsibilities Provide nursing care within the Nurse Practice Act Administer medications and treatments under RN supervision Assist with training and orientation of new staff Observe, document, and report resident conditions and changes Support development and implementation of individualized care plans Assign and oversee work of Nursing Assistants Prepare shift reports and communicate updates to supervisors Maintain resident rights, confidentiality, and safety Participate in required in service education Promote person centered care and positive resident interactions Physical Requirements Frequent standing, walking, talking, and hearing Occasional bending, twisting, reaching, and lifting up to 60 lbs Ability to assist with resident transfers and mobility Qualifications Current Licensed Practical Nurse (LPN) license in the State of Wisconsin Graduation from an accredited nursing program Long term care experience preferred Knowledge of medication administration and nursing standards Strong communication and teamwork skills Ability to supervise and coordinate Nursing Assistant assignments Must pass background check per Wisconsin Caregiver Law Valid Wisconsin driver's license (if driving is required) Why Join Lakeview?
