Job Description

Job Overview We are seeking a dedicated and compassionate RN or LPN to join our healthcare team. We have two fulltime nurses on staff currently; looking to add another part time person. Looking for 10 hours per week. Candidate would be on call every third week in rotation with 2 other nurses. On call meaning you would field all nursing related calls after hours and weekend the week you are on call. Which would include having to work the floor if floor staff called in for shifts. You are paid a generous amount for being on call every third week. If called in to work the floor you would punch in for those hours as well as the on call amount received. The ideal candidate will possess a strong clinical background and demonstrate excellent patient care skills. Majority of position would be focused on assisting the 2 full time nurses with annual assessments, completing admissions when they occur, and providing high-quality nursing care to residents in our CBRF settings. Seeking someone who has MDS knowledge and experience with PDPM. Must be able to work collaboratively with a multidisciplinary team to ensure the best outcomes for our residents. Responsibilities Assess patient health problems and needs. Updating primary physicians as needed. When on weekly on call; may need to work the floor as a caregiver if needed. Ability to provide ADLs, pass medications, complete treatments as ordered when needed to work on the floor as a caregiver. Collaborate with physicians and other healthcare professionals to coordinate patient care. Educate patients and their families on health conditions, and treatment plans. Maintain compliance with HIPAA regulations to protect patient confidentiality. Utilize electronic health records in PCC (PointClickCare) for documentation and data management. Participate in quality improvement initiatives to enhance patient safety and care standards. Qualifications Current RN or LPN license in the state of practice (WI).
MDS / PDPM
knowledge and experience is a bonus Experience in long-term care or medical-surgical nursing preferred. Knowledge of medical terminology and ICD-10 coding is highly desirable. Strong interpersonal skills with the ability to communicate effectively with patients, families, and team members. Ability to work in a fast-paced environment while maintaining attention to detail.
