Job Description

Monday - Friday. 8:00AM - 4:30PM.
Responsibilities/Duties:
Duties include but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care, including clinical reminders, health screenings, vital signs, and education, seeing patients face-to-face for acute care needs, management of flow in clinic, point of contact for Primary care needs for internal/external customers. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Assesses, plans, Implements and evaluates care based on age-specific components. Administers vaccines, places laboratory orders, and other medications as per policy and within scope of practice. Demonstrates understanding of age and population specific characteristics that affect the healthcare needs of the veteran. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Bases care on the patient's goals, provides patient and family health education including self-management, prevention, and wellness. Conducts patient education on blood glucose testing and glucometers, blood pressure monitoring, and provides patient educational material for chronic diseases. Promptly recognizes urgent or emergent patient care situations, seeks assistance, and effectively intervenes as directed when appropriate. Interacts and communicates appropriately with patients and family members, demonstrating professional behavior (i.e respect others, collegiality, ability to accept constructive criticism), effective communication and interpersonal skills. Adheres to principles of infection control, safety, and universal precautions. Possess knowledge of proper handling and pre-cleaning of reusable medical equipment. Demonstrates competence utilizing special equipment needed to perform job duties. Follows policy and procedures related to fire and disaster drills, safety and security measures. Assists in the orientation and competency validation of students, peers, and clerical staff. Maintains confidentiality of electronic, written, and/or verbal patient information. Maintains a work environment that honors diversity. Demonstrates active collaboration and communication with the PACT Core Team (Patient, Primary Care Provider, RN Care Manager, and MSA) and Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service.
