•$5,000 Sign-On Bonus!• Licensed Practical Nurse (LPN) Big Horn Rehabilitation and Care Center is looking for an experienced Licensed Practical Nurse (LPN) to join the long-term care unit at our beautiful skilled nursing facility. Big Horn Rehabilitation and Care Center is committed to providing high quality services so that every resident receives the care and attention that they need. The primary purpose of the position is to ensure the highest quality of resident care available, support staff, and establish a positive reputation in the community while delivering on the Eduro Healthcare values. Benefits 401(k) Medical, dental, vision, and life insurance Disability coverage Flexible spending account Health savings account PTO and holidays Daily pay available Eduro discount program Duties and Responsibilities Provide comprehensive Licensed Practical Nurse (LPN) care for residents Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols Monitor residents' health status and promptly report changes in condition to the supervising RN, physician, and appropriate family members or responsible parties Provide skilled nursing services within scope of practice, including but not limited to tube feedings, suctioning, catheter care, wound care, dressing changes, and other ordered treatments. Accurately complete all required documentation, charting, and reports in accordance with regulatory requirements and facility policies. Support and implement individualized resident care plans while ensuring resident dignity, preferences, and rights are respected. Observe residents closely and report significant changes in physical, mental, or emotional condition to the interdisciplinary care team Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care
