Job Summary Position Overview About Community Access Clinic Community Access Clinic is a primary care practice dedicated to accessible, relationship-basedhealthcare with a strong emphasis on preventing and managing chronic disease. We serve diverse and often underserved populations and believe that high-quality primary care is the foundation of healthier communities. Our care model prioritizes early intervention, patient education, continuity of care, and respect for every individual's dignity and lived experience. Position Summary The Medical Assistant (MA) plays a critical role in supporting providers and patients by delivering compassionate, organized, and efficient clinical support . This role goes beyond rooming patients—it is central to helping patients understand their health conditions, stay engaged in care, and navigate chronic disease management successfully. The ideal candidate is patient-centered, detail-oriented, and comfortable working with individuals managing complex or long-term health conditions. Key Responsibilities Clinical Support Prepare patients for visits: obtain vital signs, medical histories, medication reconciliation, and reason for visit Assist providers during exams and procedures as needed Perform point-of-care testing (e.g., glucose, A1C, urinalysis, EKGs if trained) Administer injections and immunizations per protocol Prepare exam rooms, maintain cleanliness, and ensure proper stocking of supplies Follow infection control and safety standards at all times Chronic Disease Support Support care for patients with chronic conditions such as diabetes, hypertension, asthma, obesity, and cardiovascular disease Reinforce provider instructions related to medications, lifestyle changes, follow-up appointments, and care plans Assist with patient education using clear, respectful, and culturally sensitive communication Help identify barriers to care (missed appointments, medication access issues, transportation, etc.) and flag them to the care team Care Coordination & Documentation Accurately document patient information in the Electronic Health Record (EHR) Track preventive care needs (labs, screenings, immunizations) Assist with referrals, prior authorizations, and follow-up tasks as assigned Support population health initiatives and quality improvement efforts when applicable Patient Experience Greet patients warmly and maintain a calm, respectful presence Help patients feel heard, safe, and supported—especially those who may be anxious or frustrated due to chronic illness Maintain confidentiality and uphold patient rights at all times Qualifications Required Graduate of an accredited Medical Assistant program Medical Assistant certification (CMA, RMA, or equivalent) or eligible to obtain Current CPR/BLS certification Basic computer skills and ability to learn EHR systems Strong communication and interpersonal skills Preferred Experience in primary care, community health, or chronic disease management Experience working with underserved or diverse populations Bilingual skills (Spanish or other languages) a plus Familiarity with care coordination or team-based care models
