Must be able to genuinely care and engage with our prospects, their families, and the residents at Town and Country Manor, and be comfortable carrying out our mission: "To demonstrate the care, love, and hope of Jesus Christ to senior adults." Duties and Responsibilities Ensures resident health information is protected and not disclosed unless by permission or with established policies and procedures. Retrieves resident records (manually/electronically). Delivers as necessary. Files information such as nursing notes, assessments, progress notes, lab reports, x-ray results, correspondence, etc., either manually or electronically into resident charts. Collects, assembles, checks, and files resident charts. Ensures incomplete records/charts are returned to the appropriate department or personnel for corrections. Ensures resident records are properly completed, assembled, coded, etc., before filing. Extracts information from records for insurance companies, Medicare, Medicaid, VA, etc., in accordance with established policies and procedures and privacy rules. Picks up and delivers medical records to designated areas as necessary. Answers telephone calls regarding inquiries about medical records. Prepares written correspondence as necessary. Files active and inactive records as per established policies. Maintains logs of specific items as per established policies and procedures. Maintains requests for medical records forms and completes as necessary. Establishes a culture of compliance by adhering to all facility policies and procedures. Complies with standards of business conduct, state/federal regulations, and guidelines. Follows appropriate safety and hygiene measures at all times to protect residents and themselves. Maintains confidentiality of protected health information, including verbal, written, and electronic communications Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Protects residents from abuse and cooperates with all investigations. Must be neat, clean, and appropriately dressed to present an appearance acceptable to residents and the public, in accordance with the employee handbook. Communicate and work cooperatively with facility staff. Respond to requests for recorded medical information and assist the Medical Records Designee with workflow. Implement the system developed by the consultant for advising all disciplines on documentation to be completed or corrected. Oversee preparation of records in answer to subpoenas, ADR's, requests, and appeals. Assist various departments with utilization review functions. Ensure Medicare patients have timely certification/re-certifications signed by the attending physician and present to the Triple Check committee at the monthly meeting. Assist the MDS coordinator regarding MDS data assessments - reentry/discharge and transmission of assessments. Perform other related duties as assigned by your supervisor.

Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must have patience, tact, and willingness to deal with difficult residents, family, and staff. Ability to work beyond normal working hours and on weekends and holidays when necessary. Must be able to follow oral and written instructions. Must be able to move intermittently throughout the workday. Effective verbal and written communication skills and ability to exercise judgement. Ability to maintain composure in stressful situations. Ability to be organized and efficient. Basic computer skills. Ability to handle all correspondence assigned. Ability to come to work as scheduled and consistently demonstrate dependability and punctuality. Working Conditions Ability to assist in the evacuation of residents during emergencies. Ability to bend, stoop, kneel, crouch, perform overhead lifting, and perform other common physical movements as needed for the position. Subject to exposure to infectious waste, diseases, and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis. Education and/or Experience Knowledge of Long-Term Care Medical Records procedures, OBRA, and Title 22. Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, or governmental regulations. Ability to effectively present information and respond to questions from managers, residents, family members, employees, and the general public. Must be able to read, write, speak, and understand fluent English. Physical Demands While performing the duties of this job, the employee is frequently required to stand, sit, walk, reach with hands and arms, grasp, stoop, kneel, crouch, or crawl, and balance physical equilibrium to prevent falling of self, others, or of objects being carried. The employee must be able to lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, color vision, depth perception, and the ability to adjust focus.
